If you are a new patient:
We require our financial policy to be signed prior to the appointment. Please click on the form below, sign it, and email it to firstname.lastname@example.org with your phone number or fax it to 786-565-9381 and we will call you to set up your appointment.
We know your time is valuable. We pride ourselves on running on time for your appointment. To help us achieve this goal, please arrive 15 minutes prior to your scheduled appointment time for registration and paperwork. For your convenience, you may print the new patient packet below, fill it out at home, and bring it with you to your appointment. Please note that if you arrive more than 15 minutes after your scheduled appointment time, you may need to reschedule. At least 24 hour notice is required if an appointment needs to be changed or cancelled. No show fees will be charged for patients who do not give us 24 hour cancellation notice, as stated in the financial policy form below.
Please bring a valid photo identification (driver's license) and insurance card, as you will not be able to be seen without them.
If your insurance has changed, please let us know before your appointment day, as we verify all insurance prior to the appointment.
If the appointment is for a minor (under 18 years old), they must be accompanied by a parent or legal guardian.
Due to the coronavirus pandemic, patients should come alone (unless requiring an aide or is a minor).
